Faith Formation Registration

Registration for 1st, 2nd, 3rd, 4th, 5th, and 6th grade students.

Registration fee is $40 per student.

If you are registering a new student, please submit a copy of the child's birth certificate and baptism certificate. If you are transferring from another parish, please submit a permission letter.

Please include your child's name in the memo line of the check or please include your child's name in the Venmo subject line.

This year we will be using GroupMe for communication outside email, please make sure you use the correct number below.

Are we allowed to take pictures of your student involved in our Faith Formation nightd that are to be used ONLY for church website, bulletin, or church videos. Their pictures will not be sold or used in any other way other than St. Alphonsus functions or promotions of St. Alphonsus like within the parish.
